Private “fee-for-service” help provided in the home is available, both medical and non-medical. Services may include personal care, bathing, toileting, meal preparation, light housekeeping, shopping, respite and more. TCARE® Screener Tool
CVCOA is now offering TCARE® (Tailored Caregiver Assessment and Referral tool) designed to provide support to caregivers. This innovative tool offers a comprehensive solution by assessing caregivers’ unique needs and connecting them with relevant services and resources.
We understand the valuable role of caregivers, acknowledging the blend of rewards and challenges caregiving entails. By providing TCARE® our goal is to empower caregivers with the knowledge and support necessary to navigate their caregiving journey effectively. Through this tool, we can connect caregivers to a network of services, support groups, and educational resources, significantly impacting their lives.
The TCARE® (Tailored Caregiver Assessment & Referral) system uses intelligent, predictive technology to precisely target and map interventions, reducing family caregivers’ burden & stress levels. Care professionals use TCARE®’s web-based system to assess a family caregiver’s social determinants of health, identify specific risk factors related to stress and depression, and create individualized, tailored care plans.
