Information and Assistance – provides comprehensive information about a wide range of community and statewide services, and can help you access them.
Options Counseling – helps seniors and family members explore long term support options.
Case Management – works closely with seniors and family caregivers to develop and implement care plans over the long-term.
Heath Insurance Counseling – provides help with questions about Medicare and other health insurance, including state programs.
Mental Health Counseling – supports seniors withmental health concerns that interfere with daily life.
Legal Services – provides seniors with advice and assistance on health care issues, housing and other topics.
Transportation – is provided through local transit providers.
Nutrition Services – provides nutritious home-delivered and community meals.
Family Caregiver Support – offers respite grants, support groups and trainings for the health and well-being of family caregivers
