Nutrition Services

                                                                                                             

A case manager or outreach worker using the Nutrition Screening Initiative (NSI) Checklist does nutrition screening. If a senior screens out to be high risk, a referral is given to the registered dietitian (RD). The RD may provide some or all of the following services:

• Phone consultation
• Nutrition material/information mailed to client
• Referral to other health professional
• Home visit
