Case Management

Case Managers visit seniors’ homes to work with individuals and families on issues such as:

  • financial difficulty
  • self-neglect
  • serious medical conditions
  • family dynamics
  • housing issues
  • mental health needs
  • nutritional status
  • fuel assistance application

A complete assessment of the individual’s situation and the development of an action plan to address issues is an important part of a Case Manager’s work. A Case Manager may see a client a few times a year, or a few times a week during a crisis situation, with the goal of supporting seniors to stay in their own homes as long as possible.
