Case Management

Case managers visit seniors’ homes to work with individuals and families with the goal of supporting seniors to remain independent in their own homes for as long as possible.

A complete assessment of the individual’s situation and the development of an action plan to address issues is an important part of a case manager’s work.

                                                                           

Issues might include:                                                          

Case mangers respect an individual's right to privacy and self-determination. They may see a client a few times a year, or a few times a week during a crisis situation.
