Sometimes changes in our lives or health can make it challenging to remain living independently. CVCOA Case Managers are here to work with elders, families and caregivers to make this possible.
Through an independent living assessment (ILA) they identify areas where help is needed. Case managers can provide information about a wide variety of programs and services so seniors and families can make informed decisions.
Together they help develop a unique plan of services to support each elder. Case managers help access these services and programs. They continue to work with seniors and their families over time, making changes to the care plan as life and health require.
