Choices for Care

Choices for Care (CFC) is Vermont's Long-Term Care Medicaid Program.

If you qualify, Choices for Care pays for care in any of these three settings: in your home, enhaced residentail care (ERC), or in a nursing home.

For those who opt to remain at home, the following services are available: case management, personal care, respite or companion care. adult day services, personal emergency response systems, assistive devices/home modifications. Note that 24-hour care is not avaialbe in the home-based option.

Approved Licensed level III Residential Care homes and Assisted Living Residences provide services such as: personal care, meal preparation, medication management, nursing overview, activities, 24-hour supervision, laundry/ housekeeping. Case managment is also provided.

Licensed Nursing Homes provide a wide range of services: personal care, meals/nutritional services, 24-hour skilled nursing, rehab and therapy, activities, 24-hour supervision, social services, laundry/houskeeping.

Eligibility:

  • Vermont resident
  • 65 years of age or older, or 18 years of age or older with a physical disability
  • Meet specific clinical criteria
  • Meet financial criteria for Vermont Long-Term Care Medicaid
